This form may be filled out by a student or a student’s parent whose religious tenets or practices conflict with the administration of all immunizing agents (vaccinations) in order to opt out of vaccinations otherwise required by all children attending school. This is an official state form and must be notarized.

This basic request form is advisable to be used when requesting a face mask religious exemption for your K-12 student in school. Parents are generally not required to justify the basis of their religious beliefs in order to be entitled to an exemption, but it must be apparent to the school that the professed religious beliefs are at least sincere. If for any reason this basic form is not sufficient for the school, or you prefer to bypass any potential need for further explanation, you can use the more comprehensive sample letter below. Note that private schools may not be under all the same legal obligations to grant your request as public schools.
